iMessage being delivered in two separate convos?!

Earlier on today I had a message/notification on my phone saying I needed to re-enter my Apple ID credentials as apparently someone had tried getting access to my iCloud. Apple locked it for me and after verifying and setting up a new password, I thought everything was back to normal.

I have of course had to enter my new password into my other devices (Mac & iPad Pro), but since doing so messages from my iPhone & Mac are now being delivered to my friends under two separate conversations? They both say 'Rich' on their end, but it seems my iPhone messages are sent to one 'thread', and my Mac messages sent to a different one.

Any help please guys would be appreciated!

I should also point out on my end, they're appearing in the single convo. So if i send a message from my mac, it instantly appears in the same convo on my phone?

Check on the settings of both devices that the default for new conversations is your phone number and that all your address are selected as receivers. Then check the contact you're sending to and look at if your Mac is sending it to their email, while your iPhone is sending to their number, or the other way around. What's probably happening is you're sending them to a different address on your phone, but since its the same person's contact, it appears as the same person
 
Generally it's because your default address is different. This forking of conversations usually is the fault of the sender, not the recipient.

E.g. If your default address on your phone is your mobile number and your default address on your Mac is your Apple ID, it will divide your single conversation into two conversations for any recipients.

As far as I can tell, my iPhone and Mac are both sending it directly to the phone number. I've selected my phone number on both devices on ''start new conversations from'' & selected all three (my number and two email addresses) under ''You can be reached by iMessage at'' - so this should work now?

It's late in the UK so I will test tomorrow, but just wanted to make sure I'm doing this right/this should fix it?

I concur with everything said here. Every split conversation I have had has always been because the send/receive recipients have got screwed up (user error).
