My wife's iPhone 5s can't seem to receive full screen effects in iMessage, which I figured was just because it is an old phone. But, she can send them. Isn't that a little odd? Are we doing something wrong?

(And no, she does not have reduce motion turned on)

You might want to try a fresh install. Had the 5s as well and could send and receive the animations. :)

Have you tried disabling and re-enabling iMessage on that phone?

I have a 5s and it works for me. I just had to make sure I had reduce motion turned off (which as you say you have already done). I played with it a couple of times and then turned reduce motion back on.
