I have this quirk that I can't figure out.
I have an iPhone 6, and this has been happening for a few weeks now.

When sending a text (iMessage...) it mostly sends "from" my phone number, but sometimes it sends it "from" my .me email address.

the setting is set to only start new conversations from my phone number. I have checked the settings on my iMac, iPad and iPhone and they are all set to start conversations from my phone number. I have turned off iMessage on all devices and turned it back on.

I know this is happening because my wife keeps getting two "threads" of texts on her iPhone, some from my email and some from my phone number.

Any ideas?
 
How bout signing out and back in on all devices? Then double check your settings? Also, I'd remove (uncheck send from and reach me at) your email address from imessage entirely just to eliminate that variable.

I'll try signing out of iCloud completely.

I'd like to keep the email address for recieving checked for sms forwarding to work. I'm wondering if sms forwarding might be the culprit

I get it but try unchecking all emails temporarily. Just trying to eliminate all variables.
