Easier said than done. It requires being able to identify spam calls in the first place, which is difficult.

They can give us an option to automatically delete voicemails from unknown callers not in your contact list, but this can create a few problems. With “silence unknown callers” in iOS 13, if you get a call from an unknown number, but it happens to be a very important call, they can at least leave a message and you can listen to it later. If Apple automatically deletes voicemails, then people could miss important messages from people.

Now I guess they can use machine learning, and use the transcription technology to learn about spam calls. That would probably be the approach they go with. But they’d probably separate them in a different folder and not delete them, just in case they get one wrong.
 
There are also carrier options, and I would prefer it to be at the carrier-level, not iOS. T-Mobile offers Scam ID and Scam Block, which for me so far has been 100% accurate. For now I only have Scam ID enabled, which identifies some unknown callers are "Scam Likely". I block those immediately and delete the voicemail. I can enable the Scam Block which will block callers identified as Scam Likely. I want to use it for a while longer to be sure about enabled that second option.

I also have voicemail to text option so if it's a number not in my contacts (marked in a red font), reading the first few words of the voicemail is enough to identify it as spam. I never want to live without voicemail to text, although sometimes the transcription is pretty funny.
