Is there any workaround, like using a VPN to download an App Store app that is only available outside your country/region?
 
Don't think so.

Need to change your country for your Apple ID. Which means you need a payment method that is good in the other country, no gift card balance in the account, cancel all your subscription. And along with payment method, will need a valid address in said country. And once that is all done, some of the apps you currently have might not be usable. And some media that you've purchased.

About the only hack there is is to setup a new Apple ID for the other country and switch between them. But as mentioned, will need a valid payment method for that country and billing address.

You can register Apple ID that belong to that country/region. However, you do need payment method. Usually it is credit card, debit card that originated from that country. You could try to get prepaid credit card that was issued from that country or get Apple gift card from that country
