My iPhone 3Gs was jailbroken and unlocked with the iPad firmware and 5.1 IOS. Using Redsn0w I had a custom 5.1.1 IPSW made. I was able to successfully go through iTunes and restore using the new Custom IPSW. But my phone rebooted in a Factory Setting. It asked me to install the original Sim -- which I don't have anymore.

Anyone know what I'm doing wrong here or what happened.

Grateful for any advice.

Sounds like the custom 5.1.1 you created its not activated.
So you need to hactivate it or use an official sim to activate and get into the homescreen.
Then open cydia and install ultrasnow to unlock.
Or better yet downgrade from the ipad bb to 05.13.04 using the latest redsnow.

That is what always happens when you restore. It returns the phone to stock, out of the box condition.
You can use Redsn0w to hacktivate the phone while jailbreaking, just uncheck Deactivate.
