A couple of questions here:

First, I have disabled "show suggested and recent apps" in the Multitasking and Dock settings. Yet it continues to show, in addition to 5 apps, one particular app, with a vertical line separating the app from the other 5. If I long press and reposition one of the 5 apps on the dock, then the other app disappears. If I open an app and then press the home button, the iPad goes back to the home screen and this app reappears in the dock, on the right side of a vertical line. This particular app is not one I placed on the dock. It is in a folder on one of the home screens.

The particular app in question appears on the dock with a small clock image in the upper right hand corner. I cannot find any information online regarding what this might mean.

Does anyone know anything about this?
 
This is a function of Siri. It learns when and what apps you use, then suggests them at the time of day you usually use them. If you don't like the feature, disabling Siri suggestions may get rid of it.. Then small clock in the upper right of the icon is a dead giveaway.

Aha, that makes sense. I was just checking the settings in Siri. I didn't even know that was there. :)
