Was there a change in iOS 8.4 (or 8.3 maybe) with volume control...specifically regarding bluetooth?
My bluetooth volume out level used to be separate from Audio/Video volume. I have a jailbreak tweak (Volume Mixer) that shows five volume levels:
My A2DP audio loudness used to be controlled by #5. Now #5 has no control over Bluetooth music and it's controlled by #2, the same as the built-in speaker and any regular 3.5mm plug in the audio jack.
#5 seems to have no effect. I haven't tried #4 on anything.
