It's relatively small, only 29MB, but I never downloaded it. It's not even the type of application that I would download. It had INSTALLED where the OPEN or UPDATE would be, but it's grayed out. Has anyone else seen this?

This is an iMessage app. And this one is likely a sticker app, which is a sticker pack used in iMessage in iOS 10.
Don't know how you install this but mine is installed by myself.
To remove it, enter iMessage app, and tap the icon similar to App Store icon. Then, tap four dot icon located at the bottom left, to bring up a panel showing all apps you have installed for iMessage. Tap that plus button to bring up iMessage App Store. Tap manage, then disable the one you don't want it to show. It will then be automatically removed from your device. However, it may remain in your purchased list and the best thing you can do is to hide it.
