In the past I would share my iphone hotspot with our two iPads. One iPad was the register (runs square app), the other iPad was the printer/display (ran a KDS app). Through the hotspot the two iPads would see each other and talk to each other.
If I bought a cellular iPad and shared it‘s hotspot with the second iPad would I get the same result?

Yep, back when I had a cellular iPad I used it as a hotspot for work conferences (for my work laptop, iPhone, and coworkers laptops). Worked pretty well and was nice to not burn out my iPhone battery. Most cellular plans, however, have fairly limited cellular hotspot limits (5-15GB) - so keep that in mind - Apple does a good job of treating a hotspot connection as “limited data”.

The gateway will be the cellular iPad so this might be a bit trickier.

For apps that communicate over the internet, then everything should work just fine.

For apps that work via LAN, I don't know if auto-discovery will work in such a setup. If you're able to manually input IP addresses in the apps that rely on LAN, then it should be fine.
