I have The Shining on the iPad, which I bought from iTunes, so everything should be in order in terms of proper chapters, etc. However, I noticed when I was scrolling through chapters on the iPad, a few of the chapters have completely random images. One of them looks like a four-pack of image art from a genius playlist, and on two other chapters there are images from The Dark Knight, which I also have on the device.
Anyone have any clue what's happening?

Not sure the cause, but it seems that some of the movie information has become corrupt. Have you tried removing all your movies from the ipad and re-adding them one by one?

I've tried that, and I'm still having issues. I've taken some screenshots to show the issue. It's now showing wrong movie art completely.
In this first picture, you see what appears to be a genius playlist, when in fact it should be the movie art for Carrie. Then you see there are two copies of the Shining, when the other one should be showing The Departed. You can see when you click on one of these, then the right art shows up.

Anyone else have this issue?

Yep, just happened to me today. I canceled a sync before it was finished and several movies became corrupt. I then synced the same movies to my ipod touch and the artwork is broken there too. Tells me the problem is in the itunes database and not the device.

What do you think the issue is? The artwork all looks fine on iTunes.

The video file is corrupt or the itunes database is corrupt. I suspect the itunes database because the video file is seldom written to and the itunes database is written to all the time.

My only idea was to delete the individual video files and then re-import them, but you said that didn't work.

Unless you have a lot of custom ratings and artwork you want to preserve, fastest solution I can think of is to delete the itunes database and start over. You can safely delete these files.

Yeah, unfortunately I've used Tune-Up for a lot of missing artwork, but ended up putting in a ton by hand since I didn't like all the choices, so I don't wanna do that all over again. Damn. :( If you end up going that route and it fixes things, please let me know.
