First off, this is my wife's iPad, so this *needs* to get figured out! :)

She listens to audiobooks (Audible and Libby) and streams video from time to time (especially when traveling) using Plex or Netflix, or Amazon.

The problem is, despite having AUTO LOCK set to NEVER, the iPad will stop playing the audio book or stop playing the movie/show and goes into lock mode. I've asked her to ensure there are no other apps running in the background. It continues to (randomly) stop playing and locks the screen.

What else could be causing this? She does use a passcode (who doesn't?)...

Thanks for any help.

What kind of iPad and what iOS version?

I‘ve had issues with iOS 11.2 on my 9.7“ Pro with the NPR app, and Podcasts where it did exactly this. But I updated to 11.3 and stopped having problems. Lately, it‘s been happening again with the same apps but it seems to be related to my aging ASK and the dodgy connection. The streams will stutter, slow down then speed up again. But if I start a stream to play without the ASK attached and lock it or let it turn off the screen, no problems

Pretty sure 11.4 and on the new iPad 9.7 like yours (not a Pro, but the new one that is pencil-capable).

What is "ASK"?

Sorry, “Apple Smart Keyboard”.

If it’s not skipping and stuttering like mine, which is due to the keyboard connection, I don’t have anything else to offer. I’m sorry! I tried my iPad with the Apple Pencil enabled but the keyboard detacched and didn’t have any problems.

If you have a Smart Cover for your iPad, try disconnecting it and seeing if the problem persists. And, regardless of whether it does or doesn’t, take it to an Apple Store. Because this is either a glitch that’s unique to your model (which hasn’t been reported here yet) or it’s a hardware problem. And they should replace equipment experiencing this kind of thing if it’s a hardware problem and it’s as new as yours.
