Hmmm, then I’d go through the usual IT stuff and see if anything helps:
- turn off/on again
- update OS to latest version (i f applicable)
- end processes that aren’t needed
- check battery health with a tool like coconut battery

If nothing helps, take it to an Apple Store if you have one close by.

Based on your description, I think you should stop using that iPad immediately. It sounds like the battery is dangerous and at risk of catching fire. The fix is most likely a new battery.
The battery temperature is stable tho. The part that heats up the most is the cpu (normal) but the lower part is also hot for example im currently typing this on it and these two parts are hot the battery is fine.

You can't feel from the outside which inside parts are getting hot. The entire case is filled with battery. That's what you're feeling. It sounds dangerous and I recommend you stop using it. As mentioned, take it to an Apple Store or call Apple for repair options.

He’s using iPad 5th with A9.

Sadly the chip is running at it’s limits.

Activating low power mode may reduce the CPU clock and heat.

Take it to the Apple Store, they will run diagnostics, but the iPad should be shutting down if It is hot.

Anyone who says that an iPad gets hot enough to "almost melt off your skin" without backing up what that means quantitatively really raises some serious eyebrows with me.

My advice: If you think it's getting too hot without a good reason, take it to an Apple Store or an Apple Authorized Provider so they can run some diagnostics on it. It'll be free.
