Do widgets appear on the home screen when the iPad is in the vertical orientation? If not, are the apps smaller like iPadOS in landscape or large like in iOS 12?

no, the widgets are landscape only. When the widgets are there on landscape the app icons are the width they are in portrait so the size doesn't change. The only difference is that there is less of a gap between the rows in order to fit them on.

So the app icons are much smaller in IPadOS than iOS 12, both vertical and landscape?

Compared to ios 12, yes considerably smaller. There is no longer an option in settings for display zoom which increases the size of the icons.
