My wife’s iPhone 15 Pro Max unlocks without having to enter a passcode or Face ID although they are both set.

She is on 17.0.3 and this issue occurred numerous times during our testing. However, once we rebooted the phone it worked as it should and it asked for her passcode.

Face ID now works too and is required to unlock it.

We will see if this happens again.

Has anyone had this issue before?

Yes, with my iPhone 14 Pro and in the beta testing of iOS 17 I did post this as feedback to Apple. They did nothing with my feedback.
That’s crazy! Someone could access our phones when we think they are locked but they really aren’t. Apple should be looking at this.

I have to say that I don’t have this issue anymore after the public release of iOS 17. But in beta testing I could in rare situations enter my phone after a restart without entering my passcode and also sim pin.

So far I can say I’ve never seen this on any phone I’ve ever had. That’s one ”feature” that is always consistent. I did find that on very rare occasions my phone could unlock my Apple Watch when it wasn’t on my wrist, but as soon as I touched the watch it immediately locked. I had to make at least 5 or 6 attempts to get this to happen but that was an intentional act. Unlocking my phone is something I probably do 30 times a day and have never seen it do so without FaceID or my passcode.
