Why do you have to press one button to pick up a call and then press another to then press speaker with all this available space for buttons? Then to end a call, I have to hit a tiny green bubble on top left of screen to end the call. I currently have an extreme fever and calling for help and support with all these extra steps is so tedious when I cant even lift my head. After years, Cook and his programmers dont find this annoying? Even when you do press the speaker button, you get a list of options to press and I sometimes hit another person's HomePod and the call goes there. What clumsy programming.

After trying to use and share my health data with others, the whole thing is confusing. Even when I share my heart rate, the optehr person observing me cant see live heart rate. It's jsut some occasional old data. I bought the new ultra watch only to be crippled wiht this junk OS sold as innovative health data. I think a person in serious condition would first drop before anyone knew his heart rate dropped.

I was always annoyed with this stuff but it becomes really hard when you are actually severely sick and I'm confident an elderly person who cant see right wouldn't be able to call for help in time wiht all these little buttons he has to find. Has Cook ever mentioned any of this stuff?

Even when driving a car, you have to answer call and then try to hit the speaker option while driving. Im more likely to cause an accident messing with Apple OS by going hands free.

Please educate me how I can change these things to have a smoother call answering experience at least.

Near full system customisation, which Apple will never allow, is the only way to fulfil your wishes and make emergency phone call smoother. With that being said, it is also hard for iPhone to know you are sick.

Regardless, I totally agree that Apple’s OS more often than not disappoints people when delving into details. The only time when you are completely satisfied with their software is when you live the life exactly as Apple prescribed. Not sure how viable that is.

You're doing it all wrong. Use Siri voice command and it will do pretty much what you want.

But this raises the question: what if someone is in such critical condition that he/she can’t let Siri do the job? What if Siri botches the command like it almost always do in such critical moment? Is Siri good enough for emergency use? So many questions that can’t get assuring answers to.

In this case, you really are using it wrong — read on before you infer insensitivity.

After trying to use and share my health data with others, the whole thing is confusing. Even when I share my heart rate, the optehr person observing me cant see live heart rate. It's jsut some occasional old data.
The health data sharing feature isn’t designed/intended for live monitoring:
The purpose of this feature is to enable users to share their Health app data (such as sensor data, clinical health records data, and self-reported data) with their healthcare providers, allowing providers to easily review user-provided health data, and potentially facilitate richer conversations.
After a user elects to share portions of their Health app data with a participating organization, any new data collected in the Health app is automatically uploaded to Apple’s Health Sharing Cloud approximately once every 24 hours, depending on factors such as whether the user’s iPhone or iPad is connected to the internet.

And, quite frankly, in known critical conditions, you shouldn’t be relying primarily on technology:
Sensor, wearable, and remote patient monitoring technologies are typically used in conjunction with video and/or in-person care for a variety of interventions and care outcomes.

I currently have an extreme fever and calling for help and support with all these extra steps is so tedious when I cant even lift my head.
Apple’s implantation is designed to contact official emergency medical services foremost, and rightfully so.

Call emergency services

Make the call on iPhone 8 or later:

  1. Press and hold the side button and one of the volume buttons until the Emergency SOS slider appears.
  2. Drag the Emergency Call slider to call emergency services. If you continue to hold down the side button and volume button, instead of dragging the slider, a countdown begins and an alert sounds. If you release the buttons after the countdown, your iPhone automatically calls emergency services.
You can also add emergency contacts. After an emergency call ends, your iPhone alerts your emergency contacts with a text message, unless you choose to cancel. Your iPhone sends your current location, and for a period of time after you enter SOS mode, your emergency contacts receive updates when your location changes.

Even when driving a car, you have to answer call and then try to hit the speaker option while driving. Im more likely to cause an accident messing with Apple OS by going hands free.
As already mentioned:
Use Siri voice command and it will do pretty much what you want.

Beyond that:

I have to hit a tiny green bubble on top left of screen to end the call.
Unless I am misinterpreting, that’s only when you minimize the call/phone app (i.e., compact mode):

If the phone app is left open (i.e., main focus), the End Call button is a large, red circle:
