Does the iPad know when it’s connected to iPhone for ceullar connectivity so that it doesn’t download app updates as oppose to being connected strictly to wifi?
 
I think that hotspot is WiFi to connected devices. I have auto update turned off on all my devices so I don’t get nasty update surprises when using cell data or WiFi hotspot.

It knows it's connected to a personal hotspot but as far as I know no difference is made between connection to an iPhone hotspot or a normal WiFi connection. iCloud photo library updates, apps/music download etc. will easily drain your data plan unless you set your iPad to not download anything in settings.
