iOS iphone os install for the app testing

One of the Apple document says, regarding the iPhone App Deployment:

" Please DO NOT install the iPhone OS before registering device UDIDs, as installation on
non-registered devices will render them inoperable."

does it mean , we need to install the iPhone OS again for the testing the application on the device or else... what does it mean?.

Also where the device UDID is to be regestered.

Please can anyone make it clear.

Why not contact apple on this, I'm sure they can clear up that confusion fairly quickly and easily.
