I have an issue that occurs with all of my bluetooth devices including Plantronics BB Fit Bluetooth headphones, a motorola bluetooth headset, and a bluetooth audio adapter in my car. When I receive a call it defaults to "phone" audio even though the bluetooth device(s) is connected. I have to manually switch the audio to the specific bluetooth device each time I receive a call. How do I resolve this?

You can potentially try playing with some settings at Settings > General > Accessibility > Call Audio Routing to see if something there might be of some help
