I have read about other people having this problem on other sites but never came across a resolution.
I was watching a youtube video on my iphone and listening to it through my earbuds. The clip ended I closed youtube and went back to the homescreen but as soon as I unplug the earbuds my ipod starts playing through my external speakers on its own. So it's bad enough I was watching a youtube video on my iphone at work but as I go to put everything up an Adam Sandler comedy skit starts playing out loud though the highest volume setting available. This pretty much always happens so I'm prepared for it but everyone around hears what it is before I can pause it.
Anyone else have this issue or know what I'm doing wrong so I can prevent it so everyone at work will not know that I'm playing other then working :)

This happened to me once during class (i didnt unplug the headphones though, it just started playing).

The iPhone has three different volumes (headphones, speaker and ringer) . What i do now is unplug the headphones in the iPod app and turn the volume down all the way. That way if it starts playing it wont make any noise. This also wont affect your headphone volume or your ringer volume (you can leave them turned up). I believe the youtube app uses this volume as well so it should silence that too.
 
I think this is definitely a software issue so a Restore should fix it. If it doesn't for some reason then you should Restore again but don't use a backup. Then you can sync from a backup if that solves the problem but the problem may occur again.
