Is my OWC PCI-E SSD defective?

I had a PCI-e SSD 480 GB installed in slot 4 in a Mac Pro 2009 5,1 for 3 and a half years.
For some reason I was forced to shut down the computer and now the card doesn't work anymore.
The first time that I booted with the recovery partition, the computer hanged. Another force shut down. Then I booted from a different hard drive and after some time the OWC SSD appeared at the desktop. But the SMART status was not OK and I had to re-initialize the SSD. However disk utility hanged and after 20 minutes I decided to force quit. I tried again and this time disk utility couldn't open the disk. I zapped the PRAM. No result. I tried the card in another Mac Pro and there disk utility couldn't open the drive either.
Can someone give me some advice?
a) installing the card in slot 2?
b) switching the two blades??
c) keep the Mac for 20 minutes in the startup screen with the recovery partitions and without selecting one?
d) something else?

Time to say goodbye to OWC. I personally will prefer a new Kingston Predator. It comes with the adaptor, 100% OOTB solution, with proper warranty. And I don't think you can feel the different than SM951 in normal daily ops. Also, it has 960GB option.

Rather than extreme speed, I think go for a more balanced solution is a better choice. If we need extreme speed, we should not stay with the cMP nowadays.

Cost is also a factor. I just paid £190 for a used SM951 512GB vs £316 for a new 480GB Hyper-X.

The "best" choice seems very location dependent. In my city, a new 960GB HyperX with warranty cost $429 (local physical store). A used 512GB SM951 no warranty is about $207 (online store from china, including shipping). So, their cost per GB is almost the same.

Very weird, I have an Accelsior too and mine suddenly stopped booting tonight but only to 1 partition of it. What is weird is if I try and delete/erase the partition it shows 'couldn't open disk' but again only on the 1 partition. The other partition (on the same Accelsior) boots fine. Any ideas on how I can wipe the other partition if Disk Utility is kicking out this error ?

To my surprise, I discovered a solution. I was unable to format the drive in the finder. A message appeared: "Couldn't open the partition map." But after booting in the recovery mode with command-R I could initialize the disk. I checked it with Disk utility and Techtool Pro and no errors were found. I installed a system on it.
I will post an update in a few days. But everything seems to be OK.
