Currently on iOS 14, if your iPhone is locked, when an incoming call arrives there is no Decline button - you have to press the power/sleep button twice to decline. There is a red decline button if the phone is not locked when the call comes in. So - I am wondering, has this changed in iOS 15, to add a decline button when the screen is locked for an incoming call? Thanks!

No change.

As before, one press of the power button to stop the ringing, two presses to dump the call. On-screen slider to answer.

You’ve answered you own question. Why add this when the physical button exists?
 
there is never gonna be just a simple button, same as why there’s no button to accept, but a slider. to prevent unintentional interaction.
There could be a slider where the knob is in the middle and sliding to the right/left does either accept or decline. That‘s how Android does or did it for a long time.

Can you slide up to go to home without declining the call?
