I listen to audiobooks with bluetooth earbuds. Often, if the earbuds are connected but not in my ears and a notification arrives, I won't hear it. Ringtones and alarms come through the speaker, but not notifications.

Android does this, not iOS. If there is a tweak to do this, I think I'd jailbreak.

Yes that is up there with some of the more stupid things that Apple thinks is the right way to have it. Not everyone that is connected to Bluetooth is driving.
