Have you tried removing your Exchange account and adding it back?

Because Microsoft was successful in luring in and then holding hostage every single one of their corporate and business customers going back years… they have essentially married the corporate business market and there has never been a viable alternative to the Microsoft Server suite of products… particularly exchange.

Apple tried and failed miserably years ago…. Likewise Novel, and a couple small others…. Very few to no one was willing to put forth the effort to migrate away from the Microsoft server products, and those who did got burned by the likes of Apple who abandoned their server product line after just getting it off the ground… remember the Sonnet xMac Mini Serve rack mount boxes?

As of April 21, 2022, Apple has discontinued macOS Server. Existing macOS Server customers can continue to download and use the app with macOS Monterey.Apr 22, 2022

This is an Outlook issue and my guess is because Microsoft never invalidates the keychain. What works for me is going into outlook, removing my account, restarting, then readding my account. I have to do this every time I change my password.

I get these regularly from time to time. I suspect that it's an Outlook issue as I can access my Outlook mail just fine.

I’m using Apple’s built-in mail app in OSX, not outlook.

Unless this is coming up repeatedly within a day or two of each other, this is normal behavior. Microsoft requires that you re-authenticate every few weeks.
