I've been organizing and fixing problems with my iTunes library for weeks now, and I started having problems with adding album art today. I've figure out a work around but I'd rather not have to deal with the problem in the first place.
File type: mp3, generally 220 kbps or better bit rate.
May be constant or variable bit rate
id3 tag v2.2 or v2.3 ( I tried both )
Symptom:
The album art doesn't appear in album or artist views as it should.
Go back in to album properties and notice that there isn't any album art .
Work around:
OS X 10.14.3.

Symptom:
- Download album art file (jpg or png )
- Open Album properties and go to Album Art tab
- Use any of these methods to add album art ( method doesn't change the problem )
- * Click and drag art file to Album info pane or
- * Click add art , browse to desired file, select image and click ok
- * Verify that album art appears in both the Album Art pane and in the thumbnail square in the top left of window. There is a green checkmark on the image.
- *click ok to save.
The album art doesn't appear in album or artist views as it should.
Go back in to album properties and notice that there isn't any album art .
Work around:
- Select all tracks of album
- Click on File->Convert to Apple Lossles to convert files to AAC format.
- Delete the original mp3 files after the AAC files are created.
- Repeat the steps for adding album art from above.
- Note that the album art is now visible in all views.
