Keep getting the grey screen of death after using computer for short time. It says computer needs to restart. Here is the report... any ideas?

Do you have any external peripherals plugged in, such as a monitor, hard-drive, etc? If so, unplug and see if the issue persists.

The kernel panic report seems to be mentioning the filesystem as root causes of the issue. As such it could be a faulty drive. Please could you download/run/screenshot the outcome of SMART Utility: https://cloudfront.volitans-software.com/smartutility323.zip

If that comes back passed, it may be the RAM. As you have the Late 2012 27", did you upgrade the RAM at any point? If you did, what manufacturer RAM did you use, and was it definitely 1600MHz?

Thanks for your help! I have never upgraded the RAM. I have started the machine while pressing "D" to get to diagnostics, and have ran the systems test which tests RAM, hard Drive, and Main Logic Board... all passed with no problems. I will run your diagnostic tonight when I get home.

I did have a USB memory stick, Apple CD-ROM, and External Hard drive hooked into the system via USB, but I have tried the system with them unhooked and it seems to be running into the same problem.

The system does run fine in SAFE MODE... so this morning I reformatted the hard drive and am installing sierra upgrade as we speak (as the reformat installed Mountain Lion). Hopefully it is just a software thing, and that will fix it. I did notice that MD_Stores was the CPU taxing object when I looked at activity monitor... isn't that associated with spotlight?

I'll report back after doing more testing tonight.

Late 2012, 27" with 2 16GB Crucial kits. The only time I ever received a Kernel Panic, I did pretty much what you've done. Blow the drive away, reinstall the OS (I think this was spring '14, Mavericks), yada yada, . . . . . Ultimately ended up doing a cross-ship with the memory manufacturer for all 32GB. Not a lick of trouble since.
I would suspect a memory module.
 
I ran the diagnostic, all was good. So I reformatted the hard drive and did a clean install of Sierra. All is good now! SHWEEEHHHH!
