It's possibly an OS corruption or more likely a hardware failure (suspect HDD/RAM or even MLB). Do you have an external OS partition you can boot into so we can run some diagnostics? Failing that, if your Mac's still under warranty, it may be best to take it into an Apple Store.

HDD smart info seems to be ok:

I just put computer to make memory test, it is 57% and no errors yet.
My mac don't have warranty no more.

If I boot into recovery mode and try to verify my main partiton with disk utility I get this information:

root drive corrupt and unrecoverable... follow the suggestion on the last message if u don't have a TM backup to recover from.

drive is probably dead, consider changing it... or backup often if reusing it.

Just had the same thing happen on my 2012 Mac mini. Same message booting into recovery and using disk utility. I had a hybrid drive setup. Luckily I had a external backup and was able to take the hdd out and erase the ssd. Copied from the backup to the ssd drive and everything seems ok. Guess it was the hhd.

It seems to be definietly hard drive problem, but I am not sure is it hardware or file system problem. If I try to copy data from hd I get errors and copying stops. What would be best way to get data out of my hard drive.
