I'm in big doubt here: I don't know if I activate the optimized battery charging and I keep the GPS being accessed at all times or if I turn this option off and leave it without the optimized battery charging.

Would this constant access to the location be access to GPS or just Cellular Towers for the system to know the approximate location of my home?
 
Optimize charging REQUIRES Location Service AND Location Services > system > system customization AND Location services > system > Significant locations to be turned ON. Also Wifi, cellular and bluetooth need to be ON.

Wifi and BT are adequate to determine if you are home.

Not exactly sure what you mean by 'constant access to the location be access to GPS or just cellular towers'. Location data is only accessed when an app needs the information. Generally an update to location services is sent to apps that request it.

If you have an app that is requesting location services in the background too much you will get an alert...

Why are significant locations important?

Keep in mind Apples main goal is not to inconvenience people and have them leave for work in the morning with a partially charged iPhone. Reducing battery wear comes second to that.

Optimized charging is utilized when the iPhone is plugged in for a time significantly longer than it needs to fully charge. To get it right iOS needs to establish your routine. To establish your routine the time its plugged in needs to be weighed against the time iOS knows it will be unplugged. And of course the location this happened.

Typically this is at home or work, but not always not for everyone. For example, say you spend the evening/night at a girlfriends/boyfriends house every night and charge your phone there. Significant Locations can establish that as "the place". Conversely say you goto a friends house for a party one night and plug your phone in, Significant Locations can establish that is "not the place".

Significant Locations seems like a privacy concern but thats because its categorizes the data making it easy for us to read. However that data is obvious, knowing where you work, sleep and get groceries is something most of our neighbors probably know. Its really a double edge sword for Apple, to be completely transparent with the data makes them look like they are tracking you. Meanwhile certain manufacturers do the same thing without telling or showing you nor do they give you a way to disable it.

Battery impact is negligible. Its just marking your significant location, iOS knows your non significant locations as well. And as long as you are on a known wifi the iPhone doesn't need to use additional corelocation services because iOS already knows where you are for apps that request location. Keep in mind, apps that want your location generally dont need precise that, weather for example just needs your city. GPS uses the most amount of energy but its generally only used for navigation apps.

My device is charging optimally every night, but it keeps showing the arrow next to the clock every time I turn it on, in addition to appearing randomly with each movement of mine. My doubts were always two: Whether these accesses would be done via GPS (navigation) or just a reading of my WiFi at home, or even from the cell tower near my home, where I would prove that I would be at home. The other doubt would be if these constant accesses would not use more battery during the day, not compensating to activate this option in favor of a longer battery life in the future. That we do not even know if such a function will really extend the battery and if it significantly.

Remembering that only Google Maps, Find My iPhone, SOS, System Customization and Important Locations are enabled in my Privacy> Location System. So the optimized charging system is responsible for accessing my location at all times.

Since I don't like to see the arrow at all times indicating that my location was being accessed, I disable the "System Customization" and "Important Locations" options each morning and activate them before connecting the device to the charger. The battery optimization option keeps working!
