After 9.2 I can't check the time and date on the lock screen because it either has music or video controls. I have not even been using music or video since I upgraded to 9.2.

A second push of the home button will show the date and time. It's been like that for ages.
Yep. At the lock screen, just press the home button once (with a non-touch id finger if you have Touch ID enabled) and it'll toggle back to the clock. Tap again for music controls. Double tap for wallet if you use Apple Pay.

The music app was closed.

Likely another app that was still open was using media in some way at some point, even possibly some pat of a page in a tab in Safari. Best way is to close out all of the apps from multitasking if you are not sure which one it might be and that should resolve it.

That's the weird thing. ALL my apps are closed.

It's probably proactive guessing you normally would play music at this place or time. Working as designed. Just follow the advice above.
