Looking for an app...

I'm trying to find out if an app exists for iOS that will do this:

a. Use my iPhone's camera to let me scan the barcodes of movies that i own.
b. Uses those scanned barcodes to form a list of movie titles.
c. This list would let me know if the scanned movies are DVD, Blu-Ray, UHD 4K or Digital Combo packs
d. This list would preferably include cover art on it, aiding in shopping for new movies (to quickly make sure that I don't already have it)

Anyone have any idea?
