mac mail send icon missing

The Send icon in Mac Mail (version 9.3) has suddenly gone missing, so I am unable to send mail in OS X version 10.11.6. There have been no updates or changes to Mac Mail or the Mac Mini computer in general. The customize toolbar does not currently contain the Send icon (see the screenshot attached to this posting). The Connection Doctor indicates that "connection and login to server succeeded." The app can receive email but cannot reply to email messages.

Thanks for any tips on how to restore the Send icon on Mac Mail.

That's the Customize Toolbar dialog for the Inbox window. The (compose) New Mail window has its own toolbar. To edit the New Mail Toolbar you have to open a New Mail window. Then you'll see the Customize Toolbar you desire.
