If I can see on the output error log, the problem seems from com.apple.kext.AMDRadeonX4000

Anyone has facing the same issue here? Please let me know. I'm starter in Open Core.

I would try booting without OCLP. Mojave and RX-580 are natively supported by Mojave. If the RX-580 is not flashed, you will not have the original boot menu.
It is difficult to debug hardware issues with OCLP in place.

Did you mean to install the original mojave and not using the oclp? Does mojave available for mac pro 5.1 without oclp? Also, how to back from oclp to original mojave? Thank you

With my mac spec with rx 580 graphic card. Could i use sonoma or big sur for my mac 5.1? Since i need xcode and latest os for software development. Does my spec compatible with it?

Also, if i upgraded to latest does it fixing my freezing issue? What the best scenario of solution which i can achieve?

Mojave is the newest supported macOS, after that you need to hack it. Monterey is the last macOS release that fully works, Ventura/Sonoma require AVX 2.0 which the MacPro5,1 Xeons do not have support for it.

I have read your Thread but I don't have any OEM GPU. I only have RX 580. Run ATS/ASD seems doesn't works either since I only have 1 GPU. Furthermore, Is there any way to find out the culprit of my issue instead of re-install the native apple mojave? Im in project right now so I cannot re-install the OS. Please anyone help for any advice.

Buy/borrow an AppleOEM GPU, ASD it's the only reliable way to diagnose a Mac Pro. Apple GT 120 is a very cheap one, Apple HD 5770 also can be found easily.

Again, OCLP is not compatible with Mojave, start removing OCLP or do a clean install. You can't run ASD with a Mac Pro that have OCLP.

After OCLP is removed, run ASD and diagnose your Mac Pro, it's very probable that your Mac Pro have a hardware problem besides just the issues caused by the incompatibility of OCLP and Mojave.

Since it's an early-2009 Mac Pro cross-flashed to MP5,1 - SMC version 1.39f5 - I bet that your Mac Pro also have a corrupt BootROM after all the crashes. It will get worse, since each crash is also saved inside the NVRAM volume of the BootROM. This is deadly if you insist using the Mac Pro as is and the BootROM will autodestruct itself.

Im in project right now so I cannot re-install the OS. Please anyone help for any advice.

No sure if any software issue yet. But at least re-seat the graphic card. Looks like not installed properly (physically). Also, which RX580? How you power it? Is it a used card? Possible mining card?
