Hi, had a good search but cannot find anyone that has had this issue. I have upgraded multiple Mac Pro 5.1 (some 2012 and some 2010) to Mojave, as well as adding X5680 CPUs and 128GB of memory, all have gone OK until this one.

After the CPUs and Memory were upgraded (fans were normal before), the fans now run at full speed... swapping the Processor Tray into another Mac and the full fans issue follows the Processor Tray. I then compared the system info of both trays in the same Mac and the "full fan" board doesn't even show a line for "SMC Version (processor tray)", see the screenshots below.

Seems you have a problem with the tray SMC micro-controller. I had this before with a corroded tray and a little after that the tray died.

There's nothing software upgradeable on the tray, no jumpers or anything to configure, so unless it's a bad contact somewhere you probably will have to replace it.

The tray did look a bit worse for wear when I replaced the CPUs. Maybe I aggravated things when I was cleaning it... a little annoying, but a lesson learned. There is an oily like residue all over the rear right components, maybe flux or maybe a capacitor leaking?

If all else fails maybe I’ll bake the board for a bit :)
