The problem appeared on one of the last updates of High Sierra — when Mac pro goes to sleep, it turns off completely. Other time it functions properly, it can be turned on for a couple of days and it is ok, even under heavy load (rendering). In safe mode it is the same, sleeping turns Mac Pro off. Installing Mojave did not help.

I reset SMC, NVRAM.

i did "log show --predicate 'eventMessage contains "Previous shutdown cause"' --last 24h" in Terminal, it says:

What it can be? I don't think that it is power supply issue, it seems to work well.

Yes, from time to time it can wake up (it happens totally random) but usually it does not. I have stopped trying and turned auto sleep off, but it is not a solution at all.

Longshot, but did you installed MP51.0087.B00 Firmware? Did you have trouble booting sometimes?

Is your current macOS installed from an upgraded previous version? Is it a clean install with Apps pulled over from TM or Migration Assistent?

I had similar problems that wouldn’t go away until a clean install onto a fresh partition, and each app installed as new. Nothing migrated from a previous install (don’t copy your old home folders over to new install).

This totally fixed all sleep an power management issues for me.

High Sierra was clean installation, and then upgraded to Mojave some days ago. Problems appeared on one of the last updates of High Sierra.

You can check two things before starting to search a hardware fault.

1) Check your BootROM for corruption, it’s common for people that had MP51.0087.B00 installed.

Dump your BootROM with ROMTool, I'll check it for you.

First boot Recovery, open Terminal and disable SIP:

Then download ROMTool.zip and dump your ROM. Compress and PM me it.

2) Do a clean install of 10.13.6 on a empty drive and check if you stil have sleep problems.
 
Things gone worse today, because Mac Pro starts switching off while it works. Three times today i started working in FCPX and it switched off completely.
I have never seen such behaviour for a year of using Mac Pro.

The further investigation of the problem, and steps i have made:

I flashed BootRom with the Rom kindly checked for me by @tsialex — it didn not help (my BootRom didn't have trash or the usual corruption).

I did clean Mojave installation, fresh system that had the same problems with sleep and switching off while working.

I did stress tests on fresh system — LuxMark Gpu and LuxMark CPU. MacPro switched off while performing CPU test.

I thought that temperature of North bridge can be too high and it forces system to shutdown. I replaced thermal paste on CPUs and NorthBridge.

It did not help, system shuts down under CPU load, on start of Geekbench 3, for example.

So, my conclusion is power supply replacing. Random shutdowns in sleep mode — shutdown EVERY time in sleep mode, — shutdowns under CPU load lead to it.
 
Something similar happened to me. It was the power supply. Took it to "genius" two times who found nothing wrong with it. Talked to the manager on the third time and was immediately told that it was the power supply.

My Mac Pro 4,1 converted to 5,1 also has issues with sleep with the official release of Mojave. It will reboot and the grey error screen appears, when waking from sleep.

Should a reset of NVRAM or SMC solve it ? However I guess that has no meaning as when I boot into High Sierra 10.13.6 from another drive the issue is not there at all.

It was upgraded from High Sierra 10.13.6 to Mojave 18A389 (issue started), and then upgraded again to 18A391 public release. . Issue still there and same behaviour. Firmware is 138.0.0.0.0 as found in the 18A389 full download installer.

A Nvidia 1050Ti card is also installed in addition to a HD7970, the Nvidia card has no driver loaded in Mojave as Nvidia has not released it yet..

EDIT: I removed the Nvidia card that has no driver loaded in Mojave and problem solved.

If this happened to me I would backup - format the drive and do a complete re-install.

Incidentally, have you run Disk First Aid on your boot drive ? Might help.

Try to boot only with one card, HD7970 if it supports Metal in Mojave. I had some issues with two cards installed in High sierra at once (Rx580 and EFI OEM card).

I also have an RX 580 in this thing. Maybe that’s doing it.

I didn’t have any problems when I first put the 580 in, but now my computer simply shuts off when putting it to sleep maybe 2 times out of 10.

I have worked with RX 580 installed almost a year without any issues. What RX model do you have? Pulse, Nitro? Does it have native support? Try to switch GPU and test sleeping again.

I can confirm that faulty PSU was an issue. I replaced it with new one and it is ok now.

Hi all! I replaced my power supply and now all is well. I had the same problem as the OP and his solution solved mine too.
