Hey guys. I'm very new to this, so please bare with me.
I own a 2012 Mac Pro and as I was doing some work on Adobe Auditions, the computer started acting up. I restarted it, and upon booting it just turned off. I've tried rebooting in recovery mode - no luck.
I took out all storage and backup drives (1 3TB WD, 2 1TB WD, and the Samsung 960 NVMe SSD), and only kept the bootable 240GB Mercury Accelsior E2 PCIe SSD Card. Only then I was able to boot in recovery mode. Upon trying to First Aid the drive, these were the details:

Repairing file system.
Volume was successfully unmounted.
Performing dsck_hfs -fy -x /dev/rdisk0s2
Checking Journaled HFS Plus Volume.
Checking extents overflow file.
Checking catalog file.
Missing thread record (id = 400668)
Missing thread record (id = 400671)

Missing thread record (id = 400706)
Missing thread record (id = 400707)
Missing thread record (id = 400708)
Invalid sibling link
Rebuilding catalog B-tree.
The volume OWC SSD could not be repaired.
File system check exit code is 8
Restoring the original state found as mounted.
Problem -69842 occurred while restoring the original mount state.
File system verify or repair failed.
Operation failed...

Any help is greatly appreciated!

I had a similar issue with my 2009 mac pro a few weeks back. I'm not entirely sure what caused the boot drive to become corrupt but possibly due to a power cut (don't have a battery backup...)

I spent about a day attempting to repair the disk but in the end, the only option was to completely wipe and start again. I had time machine backups so then just restored. Was back up and running after that and has been fine ever since.

I'm restoring the bootable drive from backup now, but just in case have all other drives out. I hope this works, as the warranty on the Mercury is long gone. Did you erase and then restore, or went straight to restoring?

A little update.
Restoring went well, everything's back to normal. Glad I had a relatively recent backup.
Now I'm just wondering: Did the problem go away or should I still be concerned?

I UPS is just as necessary for your Mac as sun glasses are for you in bright sun light, or gloves in the cold.

Lou

I UPS?

Good to hear! I completely wiped the drive - reinstalled sierra and then restored from the time machine backup. In my case i've had no problems since and it's running absolutely fine. Been a couple of weeks now. I don't know exactly what caused your issue so couldn't say wether you should still be concerned.

True - but as it's happened only once in the last 15 years - I'll happily take that risk.

Better to be prepared and ready to protect your data and investment.
The next unpredicted power fluctuation may harm not only one disk but your time machine's too, have in mind that an UPS is not something really expensive... Your Macs are more important and expensive and your data really precious.
Good Luck.:)

I know it's completely off topic, but since we started talking about this:
What are good and fairly inexpensive options for an UPS unit?

CyberPower ones are good, but buy when on sale if you can. Picked up the CP1500PFCLCD for under $130 last year.

Also using a CP1000PFCLCD, believe that was also around $130 back in 2015:

Both will show in MacPro5,1 when connected via USB, but the "Show UPS status in the menu bar" does reset to NOT being enabled every few weeks. It's not a huge deal, but worth mentioning. Shutdown options work, but neither of these will run a 5,1 for very long. Basically long enough to save your work and shutdown safely.

Look for some version of PureSinewave in whatever you buy. Basically if you have dirty power or variations, it helps a lot.

Hey, I have the same problem. Sadly, I was in the process of backing everything up and it didn’t work. Then I started having this issue. My last backup was two years ago.

Does anyone know any other solutions? I need to get my baby’s newborn pictures.

Any help is greatly appreciated!

Assuming your hard drive is not dead. To recovery the data should be easy. Just plug the hard drive into any computer and the photos should be still there.

Anyway, Disk Warriors is built for this kind of issue (HFS+) and it work really well usually. It provide boot disc to recover a corrupted OS drive as well.
