My 2009 Mac Pro 4.1 with 2.66 Quad Nehalem, 16GB RAM, two 128GB Samsung SSDs and an EFI-flashed EVGA GTX570 2560MB having random sleep issues. Second monitor working great.

First, the primary screen (Apple Cinema Display 24") goes white.

Trying again, it shows my desktop (with all windows open) but NOT my cursor. I can hear the machine going to sleep, but the GPU fans stay on. The Power-LED stays on, doesen't blik or anything.

Then again, white screen and so on.

I suspect the GPU being the problem.

I have 10.8.2 with f02-driver installed, everything works fine.
 
Have you tried replicating the problem while using another card? There can be countless issues with flashed PC cards. If the problem doesn't occur with an apple-brand card, maybe try reflashing your card with a different ROM.

Also, probably won't do anything, but try resetting PRAM.
 
Ok, funny behaviour now.

I just put my old GT120 back in to test something else, then I switched again to my GTX570. Meanwhile, I unplugged the Mac from everything, and put everything back again, same configuration now.
I just pressed sleep, then the desktop without the cursor appeared, and then it went to sleep-mode. I have no idea why, but suddendly, it worked.

Tried again, this time the Mac went directly to sleep.
I think some ghosts live in my machine, I can't explain it otherwise...
