Making a custom website with entries or poll

Hey guys,

I'm trying to make a custom website, (with a custom domain name too, I'll get to that later) but I need it to be so people can submit a form type thing to me, it's sort of hard to explain, but here is the website I need to mimic:

This is what we used last year but I need it on MobileMe

Any thoughts or suggestions?

Does MM let you upload custom content or are you strictly limited to iWeb created content?

You can put content on your MM web site by putting the files in your /web/sites directory on your iDisk.
