Are the super-accurate clipping paths for mastheads on magazine covers done with blue-screened photographs, or do they have a special program/add-on?

this can be done quite easily in photoshop. How much time you spend refining determines how good the result is. You can get programs and add-ons to help, but in my experience, if you want it done right you have to do it yourself :)
 
I always just use the pen path tool and plot my nodes. ::shrug:: Automatic ways never yielded accurate results so best to stick to improving you "click and drag" :p
