I've noticed that some of my playlists have the cloud with the slash icon saying they're not eligible for match, but all the tracks in the playlist have been matched.

I experimented with one of these and found that it was a certain track causing the problem. The playlist (not a smart playlist) had Amazon free tracks it and one particular track would cause the playlist to be ineligible, even though the track itself is matched. Also, other tracks from the same album don't cause the problem.

Any ideas on how to fix? Or is this a bug?

Obviously just a temporary fix, but it would let you use the playlist - can you uncheck the song and set the smart playlist to not include unchecked?

Thanks for the suggestion. But I would have to go through all the effected playlists and manually find the offending tracks to make that work. I'm a little too lazy for that.

I've tried restarting and running match again, that seems to be helping, but some of the playlists are still having problems. I may just need to run match a few more times to get everything ironed out.

single ineligible track

I found that I had one track in the playlist that was 64kbps. That invalidated the entire playlist from being iCloud eligible. Deleting the track from iTunes fixed the playlist and it is now available in iCloud.

Things like that you can check (to some degree) with smart playlists or the iCloud status column, have it sort the ones labeled as "ineligible" so they're easier to find.
