Microphone red dot staying on

iPhone 16 pro max running ios18. I’ve turned all apps off the light still stays on in the Dynamic Island. Swipe down in control center and it says accessibility controls as shown in the attached. Any suggestions?

found the issue, I had a couple of vocal shortcuts set up and that was causing the microphone to stay on which was also causing a battery drain plus, in apple CarPlay, when I picked the phone up the xm radio would shut off. turning vocal shortcuts off remedied the problem. It lastly was preventing the camera to record video sound in spacial audio.
Sounds like a bug fix is in order.
