Is there a card that will do this:
- drive four displays
- provide boot screen or recovery screen
- power off of the main board - 2x 6pin

This will not be used for gaming
Just lots of web browser windows open: chrome and safari, as well as light excel and basic apps like 1password, calc, apple configurator, ...
 
Both the Radeon 7950 and GTX 680 "Mac Edition" cards meet all of your requirements. If you're handy then you can buy PC versions of those cards and flash them with the Mac EFI firmware that allows boot screens.

Edit: I should add that you'd be hard pressed to find either one of those cards new. They're long discontinued, so can usually only be found used on ebay, etc. Not sure if that's a problem for you or not.

If you only need the ability to display boot screen OR recovery partition. Then almost any modern AMD card can do. In this case, stick to the Sapphire PULSE RX580 8GB should be a very safe bet.
 
My gtx 680 Mac efi has all the 4 ports working but not at the same time, max 3 monitors will work. I hope that the pulse 580 will be able to do that.

Is yours a genuine Mac edition or is it another 680 that was flashed? I ask because I saw someone mention that their 4GB 680 lost use of one of its DVI ports after a macOS update. I couldn't find other people talking about it so I'm not sure if that is just an issue with flashed 4GB cards or if even the original EVGA Mac Edition cards are affected.

if that's the case, then OP should probably stay away from the 680. I did find reports of people successfully using 4 monitors on the Radeon 7950 though.
