As stated in title, if my AirPods 3 are connected to my MBP but then I remove them and put them in their case, music starts playing on my iPhone 14. This has persisted through multiple recent iOS updates. Anyone else encountered this? Potential cause/fix? Thanks.

By what I underlined above, do mean the same music you were listening to on your MBP? Or were you listening to something completely different on your MBP than what starts playing on the iPhone once you disconnect the AirPods from your MBP?

No, the last song that was playing on my iPhone, often from many days earlier.

have you tried forgetting, resetting, and re-adding them?
sometimes can clean up some weirdness.
