Yep, it's a very simple LockInfo theme which is not using Lockscreen Clock Hide or a custom slider.

There are probably dozens if not a hundred variations of the LockScreen theme.

I have seen dozens of different lockscreen themes and I have not seen that version until today. Good luck finding that exact setup.
