Just noticed that nested rules appear to have disappeared in the iTunes 10.4. Since iTunes 9, you've been able to add nested rules to smart playlists using an ellipsis (...) button in the smart playlist info window - but now we've gone back to just standard flat rules.

Anyone else notice this, or am I still being dense?

Just found the same problem myself. Can't find anything on t'internet either.

Also, in previous versions you could open video in a new window by default but that option seems to have disappeared with 10.4.

Because they are a progressive company by nature. While Adobe still sticks around with the zillion random old commands people developed over the years in PS, Apple releases Finul Cut X completely changing EVERYTHING. In the name of progress.
 
Just Alt-Click on the (+) button.

I have been struggling with the same question and finally came to find out the solution by accident: Just press the alt/option key and then you wil see the (…) button replaced by the (+) button.
