I'm looking to buy my first mac and I stumbled upon two options:
either a new 2018 13" tb MBP i5/16gb/256 or a used/refurbrished 2016 base model 15" MBP.

I'm a junior design student and the stuff I'll be running will be mainly adobe programs like Ps, Illustrator, InDesign and Premiere Pro. I also need to run Rhinoceros and adobe dimension for 3d modeling.
I know the 13" lacks a dGPU but how much does that matter compared to an older 15"? (considering the 8th gen i5 is quite powerful)

I was looking to buy a computer that can hopefully last me another 4 years or so and these two options are pretty much what I can afford...

Thoughts?

Think again whether a Mac is the best option. For the applications you have listed, a Windows machine will be better bang for buck. You could get a lightweight workstation from Lenovo for example, or a Dell XPS 15...

Nice insight! However I was trying to get a mac for the longevity of the machine, and pretty much everyone at school uses mac so transferring files and data would be optimal. I already own a windows machine but it has run it’s course and it’s now time to change. With a mac I could also dual boot in windows in case i need to use it (some say rhinoceros is more optimized for windows).

The first one is the better option for your needs. The jump to quad core is a big deal, so get that and enjoy!

I think the best of both worlds would be a refurbished 15" 2017. A lot more power than the 2016, (somewhat) improved keyboard.

I think Apple still sells them refurbished?

I'd get the 15" model with a dgpu. Sounds like you will be doing some 3d modeling and the added additional gpu will be nice to have. Plus you don't know what you will be doing with it in a few years. Try to get a 2017 refurb 15". Also, the larger screen size will be nice.

Are you sure? A new 13" 2018 with 16GB is $1,999.

The 15" 2017 refurb (naturally, with 256GB and 16GB) is $1,909.

Go figure

I live in Italy, so the lowest price that I found for the 13" TB 16gb is 1900€ (minus taxes is around 1550€) whereas the 15" 2016 base model is around 1800/1900€ but I can't get a refund on the taxes (because of a policy about used/refurbished items) and I will end up paying more.
For a used 15" 2017 I would have to pay around 2100€ without the taxes refund and unfortunately, I can't afford it.
I apologize for not giving enough context earlier...

I second this. I have also heard that those apps are more optimized for Windows, so they would probably run better on Windows. I don't know coz I don't use any of these apps, but ...

Some of the new Dell XPS machines look amazing from what I've seen.

I apologize to everyone but I forgot to specify that for the 2016 15" base model I would have to pay around 250/350€ more. At this point is the radeon pro 450 worth it?

I saw some Cinebench R15 scores and they are both around the 650/700 mark. Excuse my ignorance but does this mean that their "real-world" performance is pretty much equal?

The 3D stuff I do is not that heavy but do you think that the lack of a dGPU will penalize me greatly over the next years?
