Is this a sign of a hardware fault?

Everything seems to be working fine but recently there's been no chime at start-up.
 
Speakers that have their volume on.
I've had these speakers nearly 2 months and haven't noticed a missing chime before.

I had the same problem, i think i unplugged the speakers, pressed the volume up button, and it should work

Do you have your speakers on mute? If so it will mute the chime on reboot, even if you don't have the speakers mute on boot camp.

Try unmuting the sound in Mac OS to see if this helps.

In order to hear the start up chime, the volume has to be changed at least once before it is shut down and it cannot be muted. The next time you start up, you should hear the sound.
 
reset your PRAM by holding "command-option-P-R" on startup and keep holding until it boots again. this will set your chime to full volume. then turn your volume down and restart again. it should go back to an acceptable level.
