I have a reference design (single blower fan) MSI brand Vega 64. No abnormal fan noise at idle for my setup. In fact, I can hear my Mac Pro's various fans over anything the GPU is putting out since I raised everything using Macs Fan Control.

I've heard of some people having issues with the fans in Vega 64s in these machines. A search would probably turn up more info.

All your vega 64 are ok?

The way to solve it is to sell it and replace it with a Vega 7.

Really, I used to have the standard version, after that I got the All-Aluminium limited edition. The short version of the story,.. the default duct cooler is junk, no matter how you spin it. It will never be quiet and it will never cool. Sell you junker as fast as possible. The three fan Vega is so much better, you wouldn't believe it. But then again, there is the 5700 now if you are into OPEN Core....

Yes I'm in opencore, and I'm thinking about an RX5700XT, I use it almost for gaming in win10, so I think I can go with the shapphire nitro one or msi.
