Note Disappeared?

I got hired as a teacher and with that I needed to have a lot of already set up accounts on different websites with my school with passwords they have given me. I had compiled all those usernames and passwords given to me and put them on a note in my iPhone (5) about a month ago updating the info with anymore passwords given to me for different sites. I was doing something on that note this morning when my phone suddenly blinked and closed the notes program. When I tried opening it it showed my note as it was and then it literally got deleted by itself in front of my eyes. I didn't freak out because I have the recently deleted notes feature but when I went there, it did not show the note that got deleted, and I have no idea where it went or how I can get it back.

Did you backup your iPhone 5 to your PC or Mac? Or iCloud?
