My wife needs to keep a journal of dr. appts, findings etc. I know about Note taking on the iPad, but was wondering if there was a good app for iPhone given its small size. Most likely just note taking, but would be good if we could upload to computer.

Just not sure the iPhone size makes this possible.

Evernote - they have a great iPhone app, an app for the computer, or you can use their website.

One of the best notes apps out there. Can add locations, photos, etc. along with great organization options.
 
Evernote's interface lends itself pretty well to typing out fairly lengthy notes on your iPhone (largely due to the presence of easy-to-access formatting options like bullet points). I am not sure how useful evernote is if you want to dump in tons of small notes though. I use it more as a to-do list cum planner.
 
Best note taking app for quick notes to me is simple note. Also syncs with your PC and iPads and is free.
