iOS 10.1.1
iPad Air (cellular) & iPhone 6s+

I make notes in the Apple Notes app on my iPad, and they don't show up on my phone, even days later. I have all iCloud options turned on, on both devices. What am I doing wrong?

On the same devices, Google Keep syncs almost instantaneously.

You are discovering the quirks of the iCloud ecosystem. That issue would probably be resolved, for a while, by signing out and back in on all devices. You should probably first figure out a way to back up what hasn't been synced, if anything, to iCloud (the web site).

Also make sure you're saving the notes in iCloud and not locally in the app.

This. There are two 'folders' on each device. iCloud and 'On this iPhone/iPad/Mac'. Double check the save is going to iCloud. When you first set up a new iOS device or Mac, default is local save.

As I said, I'm only using iCloud.

Do they show up on iCloud.com? Are the notes in the iCloud folder of your notes? Just because the toggle is off doesn't mean there isn't a local save on your phone and something is preventing it from syncing. I have that toggle off and I have an iPhone and iCloud folder in my notes app.

A note from my iPad last night is not, but a test note I did on my iPad earlier today is. Neither shows up on my phone. I'm as confused as ever.

That is very bizarre. I'm assuming you have, but have you toggled notes under Settings -> iCloud on both devices? The only thing else I could suggest is reset network settings to see if that triggers it.
